HOUSE OF HOMES
REFERRAL APPLICATION

This Housing Application Referral form is intended to be completed by a service worker within Elgin County on behalf of their client. Fresh Start Support Services will compile a waitlist and prioritize community referrals for review and acceptance for residency.

Please read the following information before completing the

application:​

  • The House of Homes is an apartment building with 12 fully furnished 2-bedroom units

  • To live in the House of Homes, clients must be a pregnant or parenting mother with children ages 4 and under, only two children per unit

  • Clients must be self-containing and fully independent (House of Homes is not supervised living)

  • House of Homes is transitional housing

  • Clients must be willing to work with a Fresh Start staff to support your long-term goals

  • Parenting and life skills groups programs will be offered
