​Please use this form to let us know about any maintenance issues or repairs needed in your apartment or within the House of Homes. Please provide as much detail as possible so our team can understand the issue and respond appropriately.

For urgent maintenance concerns that require immediate attention, please contact Fresh Start Support Services directly rather than submitting this form.

 

Please do not use this form for emergencies. If there is an immediate threat to your safety or significant damage occurring, please contact staff directly using the House of Homes emergency contact procedure.
